0-25 Together and Preparing for Adulthood

    Please note: this meeting is for Parents/Carers of Children and Young People (aged 5-18 years) with Special Education Needs and Disability.

    Garth Taljaard – Service Manager, 0-25 Together Service, Hertfordshire County Council will be providing an overview of the service alongside Preparing for Adulthood and the event will cover the following: 

     Part 1: 

    •  General Information about 0-25 Together as a Service 
    • The process of a Child & Family Assessment 
    • Information about what you can expect after an Assessment has been completed. 
    • Our approach to Carers Assessments 
    • Information about Short Breaks including Direct Payments and Home Care
    • Information about How you can access 0-25 Together 

    Part 2: 

    • The second part of our presentation will focus on how 0-25 supports young people preparing for adulthood (PFA) including providing you information about: 
    • Our newly formed PFA Team 
    • How we assess and support young people approaching adulthood 

     There will be an opportunity to ask questions and share the issues you face as a carer.
