Our Carer Support Hubs allow you to meet with other carers, share information, support each other and chat over a cuppa.
We will be joined by a Social Prescriber to explain what they do and how they can help you. A Social Prescriber is there to support patients with anything non clinical. This could include social isolation, issues with local authorities, and signposting.
